We collect only what you type into our forms, we use it to contact you about care and, if you choose, send our email newsletter, and we never sell it. This site carries no advertising trackers and no third-party analytics, and we set no cookies of our own. The one exception: our clinics page shows a map provided by Google Maps, which may set its own cookies when it appears. We count visits in aggregate using cookieless analytics software we host ourselves; it identifies no one.
When you choose to submit a form, we receive what you enter: on the eligibility form, your name, phone number, email address, ZIP code, Medicare status, and the health conditions you select; on our call-back forms, your email address and (optionally) phone number; on the newsletter form, your email address. If you arrived from an online ad, we record the ad-click reference from your link so we know which outreach is working — it identifies the ad, not you or your browsing. As a security measure against automated spam, submissions include how long the page was open before you pressed the button; this number is not tied to your identity.
We do not sell, rent, or trade your information — to anyone, ever. We run no third-party analytics or advertising scripts, set no cookies of our own, and build no advertising profiles. We do not share your information with ad platforms. One page includes an embedded map: our clinics page displays our locations using Google Maps, which loads from Google as you scroll to it and may set its own cookies, under Google's privacy policy. The map shows only our locations — it receives nothing about you from us. Our visit statistics come from Plausible, an analytics tool running on our own servers: cookieless, aggregate (pages viewed, referral source, approximate region), never shared with anyone, and never tied to your identity.
Form submissions are emailed to our clinical team so a local person can call or write back. Eligibility requests are also stored in our waitlist to track follow-up and may create a prospective patient record in Healthie, our care-management system. Service providers for these tasks include Amazon Web Services (website hosting, waitlist storage, and email delivery), Google Workspace (our team's email), and Healthie.
The eligibility form requires permission to contact you by live phone call and individual email about your eligibility request and waitlist status. Separate, optional checkboxes let you choose text messages about that request and our email newsletter. You can submit the form and receive care without choosing texts or newsletters. We record each choice, its wording, and when it was received with the contact details you provided. If you choose to answer the optional “Where did you hear about us?” question, we include that response in the staff notification and waitlist record. These permissions do not enroll you in care or authorize automated voice calls or promotional text messages.
For optional text updates, message frequency varies and message and data rates may apply. Reply STOP to opt out, or contact our team for help or to stop messages. You can unsubscribe from newsletters at any time using the unsubscribe option in the email. You can also ask us to stop calls or emails by telling the person who calls, replying to an email, or contacting us using the details below.
This policy covers the website. When you become a patient, your health information is protected under the Health Insurance Portability and Accountability Act (HIPAA) and our clinical privacy practices, which your clinic will provide.

Our services and website are directed at adults. We do not knowingly collect information from children under 13; if you believe a child has submitted a form, contact us and we will delete it.
