We collect only what you type into our forms, we use it only to contact you about care, and we never sell it. This site carries no advertising trackers, no third-party analytics, and no cookies.
When you choose to submit a form, we receive what you enter: on the eligibility form, your name, phone number, ZIP code, Medicare status, and the health conditions you select; on our call-back forms, your email address and (optionally) phone number; on the newsletter form, your email address. If you arrived from an online ad, we record the ad-click reference from your link so we know which outreach is working — it identifies the ad, not you or your browsing. As a security measure against automated spam, submissions include how long the page was open before you pressed the button; this number is not tied to your identity.
We do not sell, rent, or trade your information — to anyone, ever. We run no third-party analytics or advertising scripts, set no cookies, and build no advertising profiles. We do not share your information with ad platforms.
Form submissions go by email directly to our clinical team so a local person can call or write back — that's the entire pipeline; we don't keep marketing databases. The service providers that process this data on our behalf are Amazon Web Services (website hosting and email delivery) and Google Workspace (our team's email), each under agreements covering the protection of health information. Beyond that, we disclose information only if the law requires it.
This policy covers the website. When you become a patient, your health information is protected under the Health Insurance Portability and Accountability Act (HIPAA) and our clinical privacy practices, which your clinic will provide.
